ABSTRACT

Tripterygium wilfordii is widely used in the treatment of immune system disease and has a remarkable curative effect. Triptolide and Tripterygium glycosides are the most commonly used active ingredients in clinical practice, but their treatment window is narrow and there are many side effects. The damage involves the reproductive system, blood system, cardiovascular system, digestive system, etc. Based on clinical observations and literature summaries, the symptoms of adverse reactions mostly occur in the digestive system (liver and gastrointestinal tract). Relevant scholars have launched a lot of studies of the manifestations of liver injury induced by T. wilfordii and the mechanism of liver injury. The mechanism is mainly related to liver cell apoptosis, induction of oxidative stress, immune injury, excessive autophagy of liver cells, abnormal fatty acid metabolism, and abnormal enzyme metabolism in liver tissues. This article reviewed and summarized relevant literature on gastrointestinal injury caused by T. wilfordii, but there are few studies on the manifestations and mechanisms of adverse reactions, which still need further research by scholars. In addition, this article also summarized the research on how to reduce toxicity and enhance efficacy of prescriptions prepared from T. wilfordii in the digestive system, mainly involving compatibility with western medicines (Methotrexate, Leflunomide, Iguratimod, etc.), use along or combination with Chinese medicines (single Chinese medicine, Chinese medicine monomers, and Chinese medicine compounds), acupuncture and moxibustion (electroacupuncture and moxibustion), dosage form improvement (glycol plastid gel, self-dissolving microneedle, solid lipid nanoparticles, gastric floating sustained-release capsules, etc.), processing (steaming, stir-frying, radish seed processing, money grass processing, licorice processing, etc.), and other methods to reduce toxicity. To sum up, this article analyzed the manifestations, mechanisms, and methods of reducing toxicity and enhancing efficacy of T. wilfordii-induced liver injury and gastrointestinal injury by sorting out relevant literature, in order to provide a reference for the clinical application of T. wilfordii and some research ideas for the future in-depth study of T. wilfordii-induced digestive system injury.

ABSTRACT

OBJECTIVE To provide a reference for comprehensive quality evaluation and control of the effective parts of Dracocephalum moldavica (EPDM). METHODS A total of 10 batches of EPDM were prepared, and chemical information of EPDM was collected by HPLC-Q-Exactive-MS. EPDM components were identified by literature search, database comparison and manual analysis. HPLC fingerprints of 10 batches of EPDM were established by using the Similarity Evaluation System for Traditional Chinese Medicine Chromatographic Fingerprint (2004 A edition); the similarity evaluation and common peak identification were carried out, and the contents of 5 index components were determined by HPLC. RESULTS A total of 11 compounds in EPDM were identified. The fingerprint similarities of EPDM samples from 10 batches were all above 0.96. Among 11 chromatographic peaks, 5 peaks were identified, such as luteolin-7-O-β-D-glucuronide(LG), apigenin-7-O-glucuronide(APG), rosmarinic acid(RA), diosmetin-7-O-β-D-glucuronide(DG), tilianin(TL) . The results of the quantitative analysis showed that all above 5 components had good linearity (R2≥0.999), and the average recoveries were in the range of 95.12%-98.37%. The contents of LG, APG, RA, DG, TL were 21.268 3-29.243 9, 6.365 4-7.771 7, 37.327 4-45.671 2, 17.169 9-21.985 9, 66.940 4-91.206 3 mg/g, respectively. CONCLUSIONS The established method of component identification, fingerprint and content determination is stable, feasible and reliable, which is suitable for the comprehensive quality evaluation and control of EPDM.

ABSTRACT

Objective:To provide insight into the clinical characteristics, therapy and prognosis of patients with radiation-induced brainstem injury after radiotherapy.Methods:From August 2013 to September 2021, 13 patients with radiation-induced brainstem injury were included in this study at Sun Yat-sen Memorial Hospital, and the general information and strategy of radiotherapy were collected. A retrospective analysis was adopted to investigate the clinical and radiological characteristics, treatment and clinical outcomes.Results:There were 13 cases in total, including 4 females and 9 males. The onset age of enrolled patients ranged from 29 to 66 years with an average of (48.15±10.23) years. The median dose targeted at tumor area was 70 Gy with in 33 fractions. The median interval between radiotherapy and the diagnosis of radiation-induced brainstem injury was 24 months. The common clinical manifestations included dysphagia, bucking dysarthria, dizziness and problem with balance. The MRI radiological features were hypointense on T1WI, hyperintense on T2WI, and irregularly enhanced with contrast. The median follow-up was 45 months. Seven cases got improvements, while 3 had no obvious efficacy and 3 cases died. No significant differences in prognosis were observed between those received traditional glucocorticoid and those received bevacizumab treatment( P=0.079). Conclusions:The common symptoms of radiation-induced brainstem injury were symptoms of posterior cranial nerves injury and ataxia. Lesions mostly happened in pons and medulla, with hypointense on T1WI and hyperintense on T2WI. Half of the patients have improved after treatment. There was no significant difference in prognosis between glucocorticoid and bevacizumab treatment.

OBJECTIVE To establish the method for t he content determination of 5 active components in Yixin badiran jibuya granules and their fingerprints. METHODS High performance liquid chromatography method was adopted to determine the contents of luteolin- 7-O-β-D-glucuronide(LG),apigenin-7-O-glucuronide(APG),rosmarinic acid (RA),diosmetin-7-O-β-D-glucuronide (DG)and tilianin (TL)in 10 batches(No. S 1-S10)of Yixin badiran jibuya granules. The fingerprints of 10 batches of Yixin badiran jibuya granules were drawn by Similarity Evaluation System of Chromatographic Fingerprint of Traditional Chinese Medicine(2004 A edition ),and similarity evaluation and cluster analysis were also performed. RESULTS The contents of LG , APG,RA,DG and TL in 10 batches of Yixin badiran jibuya granules were 0.279 5-0.449 9,0.082 4-0.135 3,0.184 8-0.472 1, 0.149 0-0.332 6,0.311 2-0.623 3 mg/g,respectively. A total of 13 common peaks were demarcated in the fingerprints and were identified as LG (peak 2)APG(peak 6),RA(peak 7),DG(peak 8),TL(peak 11). The similarity ranged from 0.598 to 0.990. The results of cluster analysis showed that S 6-S10 were clustered into one category and S 1-S5 were clustered into one category. CONCLUSIONS Established method for content determination and fingerprints can be used for the quality control for Yixin badiran jibuya granules.

ABSTRACT

Objective The aims of this study were to analyze the clinical characteristics and laboratory test results of stageⅣ lung cancer patients with Pulmonary thromboembolism(PTE),and to find out the risk factors for pulmonary thromboembolism. Methods A total of 70 patients with stage IV lung cancer were selected from the First Affiliated Hospital of Nan Chang University from January 2011 to October 2017. Blood routine,blood biochemistry,coagulation function,tumor markers(CEA,CA199,CA125, NSE,Cyfra211)and multi-slice spiral CT pulmonary angiography(CTPA)were collected in these patients. Univariate analysis was applied to compare the clinical features and laboratory tests between PTE and non-PTE groups. Multivariate logistic regression analy-sis was applied to explore significant risk factors of PTE. Results Univariate analysis showed that serum albumin,blood leukocyte, neutrophil percentage,increased Cyfra211 and abnormal tumor markers were risk factors for PTE in patients with stage IV lung canc-er. Multivariate logistic regression analysis showed that the number of abnormal tumor markers ≥4(OR=7. 016,95% CI:1. 916 ~25. 686)was an independent risk factor for PTE in stage IV lung cancer. Conclusion The number of abnormal tumor markers is an independent risk factor for pulmonary thromboembolism in stageⅣlung cancer. When the number of abnormal tumor markers is≥4, it is necessary to highly alert the possibility of stage IV lung cancer with pulmonary thromboembolism.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect differences among the comprehensive therapy, acupuncture and oral mecobalamine for senile sensorineural hearing loss.</p><p><b>METHODS</b>A total of 126 senile sensorineural hearing loss patients were randomized into a comprehensive group, an acupuncture group and a western medication group, 42 cases in each group. After removing and discontinuing cases, 40 cases in each group were included in the analysis. Acupuncture, acupoint injection and auricular point pressing were used in the comprehensive group. Acupuncture for 2 courses was at the affected Tinggong (SI 19), Tinghui (GB 2), Yifeng (TE 17), etc, once every other day, 10 times as a course with one week rest between 2 courses, 3 times a week. Acupuncture as the comprehensive group was applied in the acupuncture group. Mecobalamin tablet was prescribed orally for 2 courses in the western medication group, 3 times a day, 0.5 mg a time, 3 weeks as a course, one week at the interval. Pure tone hearing threshold value and tinnitus grading were observed before and after intervention in the three groups. The clinical effects were compared.</p><p><b>RESULTS</b>After treatment, pure tone hearing threshold value and tinnitus grading were better than those before treatment in the three groups (all <0.05), with better results in the comprehensive group than those in the other two groups (all <0.05). The total effective rate of pure tone hearing threshold value was 82.5% (33/40) in the comprehensive group, which was superior to 67.0% (27/40) in the acupuncture group and 62.5% (25/40) in the western medication group (both <0.05). The total effective rate of tinnitus was 83.3% (30/36) in the comprehensive group, which was superior to 69.7% (23/33) in the acupuncture group and 57.1% (20/35) in the western medication group (both <0.05).</p><p><b>CONCLUSION</b>The clinical effects of comprehensive therapy on senile sensorineural hearing loss are superior to those of simple acupuncture and oral administration of mecobalamin in terms of pure tone hearing threshold value and tinnitus grading.</p>

AIM: To clarify the modulation of autophagy in ischemic penumbra by hydroxysafflor yellow A ( HSYA) after cerebral ischemia/reperfusion ( I/R) injury.METHODS:Male SD rats subjected to transient middle cere-bral artery occlusion for 90 min were randomly divided into 4 groups:sham-operation (sham) group, cerebral I/R (I/R) group, I/R+HSYA group and sham+HSYA group.Modified neurological severity score (mNSS) was used to evaluate the neurological deficits of the rats at 6 h, 1 d, 3 d and 7 d after reperfusion , accompanied by the detection of autophagy , apoptosis and mRNA expression of IFN-βin ischemic penumbra .RESULTS:Compared with sham group , upregulation of LC3-Ⅱand degradation of SQSTM1/P62 were observed in I/R group, indicating the activation of autophagy after I/R.The activation of autophagy in I/R+HSYA group was significantly enhanced by HSYA on day 1 and day 3, and inhibited on day 7, as compared with I/R group (P<0.05).Besides, the mRNA expression of IFN-βin I/R group was significantly upregulated at 6 h, then downregulated on day 1 and day 3, and returned to basal level on day 7, as compared with sham group (P<0.05).In I/R+HSYA group, the mRNA expression of IFN-βwas significantly upregulated on day 1 and day 3, accompanied by the inhibition of apoptosis on day 3 and the significantly decreased mNSS from day 4, as compared with I/R group (P<0.05).CONCLUSION:HSYA alleviates cerebral I/R injury by dynamically modulating the activation of autophagy and the expression of IFN-βin ischemic penumbra .

ABSTRACT

Objective To investigate the surgical reparation of large coloboma raw surface after facial tumour resection in elderly patients.Methods According to the position and characteristics of tumor as well as the age and tolerance of the patients, full thick skin graft, the skin flaps with subcutaneous pedicle and free skin flap were designed and used in the reparation.Results 24 cases with large coloboma raw surface (5 cm×7 cm-12 cm× 16 cm)were treated by the utilization of three approaches after tumor resection.The large coloboma raw surface in all patients achieved the healing with satisfactory appearance.Conclusions After facial tumour resection, the large coloboma raw surface can be repaired by using the skin graft, skin flaps after tumor resection or free skin flap if designed reasonably.The procedure of operation is simple and the therapeutic effect is satisfactory.

Objectives To explore the effect of mibefradil, a kind of novel calcium channel antagonists, on proliferation of human pulmonary artery smooth muscle cells (HPASMCs) induced by platelet-derived growth factor (PDGF). MethodsHPASMCs were culturedin vitro, and randomly divided into control group, PDGF group, Mib group, and PDGF+Mib group. The PDGF group was stimulated by 25 ng/ml of PDGF. Mib group was intervented by 10 μmol/L of Mib. PDGF+Mib group was treated by PDGF and Mib. The reproduction rate in 48 hours and 72 hours were detected by MMT. Cell cycle was detected by lfow cytometry. Objective] Discuss the“five seasonal organs”theory of The Inner Canon of Huangdi, especially the seasonal issues of the spleen and preliminarily speculate the formation period. [Methods]By means of literature and historiography methods, information before The Inner Canon of Huangdi would be gathered, especially“The Book of Rites·on Order”,“Guanzi·Seasons”,“Annals by Lv·Twelve Ji”and so on. On this basis, we can discuss the different perspectives of “five seasonal organs”theory and the seasonal issues of the spleen of The Inner Canon of Huangdi, and preliminarily speculate the theory’s origin, evolution, and formation. [Results]“Six periods connected to six internal organs”theory is more concerned with the meaning of anatomy, although it is early view of the“seasonal organs”theory, but six or twelve has its specific cultural origins;“Eight wind connected to eight internal organs”theory is affected by Zhou Yi;“four periods connected to four internal organs”theory couldn’t explain attribution of the“soil”and its role, it belongs to the early stages of Qi argumentation of the“seasonal organs”theory after the introduction of the Five Elements Theory;“five periods connected to five internal organs”theory reflected the tight integration with the theory of five elements and balanced attachment features; the theory which has the most theoretical and clinical meaning is“four periods connected to five internal organs”theory. [Conclusion] The different perspectives of“five seasonal organs”theory and the seasonal issues of the spleen of The Inner Canon of Huangdi are results of a number of philosophical thoughts absorbed by different doctors, but through analysis of the relevant literature, the core ideas can be briefly summarized, it can be generalized that the spleen is located in the center, connected at any time, behalf of governance after each season ’s eighteen days.

ABSTRACT

Eisenmenger syndrome (ES) was considered as a surgical contraindication in the past time. Only a few patients got lung transplantations or heart-lung transplantations due to lack of donors. Most of the patients had to choose conservative treatment to prolong their survival time. Recently, some clinical trials indicated targeted therapies in pulmonary arterial hypertension (PAH) were effective for the treatment of ES. This article reviewed the recent advances in this area.

ABSTRACT

The purpose of using brain-computer interface (BCI) is to build a bridge between brain and computer for the disable persons, in order to help them to communicate with the outside world. Electroencephalography (EEG) has low signal to noise ratio (SNR), and there exist some problems in the traditional methods for the feature extraction of EEG, such as low classification accuracy, lack of spatial information and huge amounts of features. To solve these problems, we proposed a new method based on time domain, frequency domain and space domain. In this study, independent component analysis (ICA) and wavelet transform were used to extract the temporal, spectral and spatial features from the original EEG signals, and then the extracted features were classified with the method combined support vector machine (SVM) with genetic algorithm (GA). The proposed method displayed a better classification performance, and made the mean accuracy of the Graz datasets in the BCI Competitions of 2003 reach 96%. The classification results showed that the proposed method with the three domains could effectively overcome the drawbacks of the traditional methods based solely on time-frequency domain when the EEG signals were used to describe the characteristics of the brain electrical signals.

ABSTRACT

Caveolae are cholesterol-rich pits on the surface of cell membrane,and participate in various physiological processes.Caveolin-1 is an important membrane protein constituting caveolae.It exerts biological effects through binding a variety of signaling proteins.In the pathophysiological processes of cerebrovascular disease,caveolin-1 participates in maintaining the integrity of blood-brain barrier and stability of atherosclerotic plaques,and has regulating the release of inflammatory mediators and neuroprotection.This article reviews the associated advances in research on the correlation between caveolin-1 and cerebrovascular disease in order to provide a new idea for clinical therapy.

Affective computing is an effective tool of revealing the affectiveness of human being in order to understand the world. Our research of affective computing focused on the relation, the generation and the influent factors among different affections. In this paper, the affective mechanism, the basic theory of affective computing, is studied, the method of acquiring and recognition of affective information is discussed, and the application of affective computing is summarized as well, in order to attract more researchers into this working area.

ABSTRACT

In the treatment of hiccup in the elders by acupuncture plus ear-point embedding method, main acupoints Neiguan (PC 6), Zusanli (ST 36) and Tiantu (CV 22) were treated. Qihai (CV 6) and Taixi (KI 3) were added for those with weak constitution due to old age and severe illness, Zanzhu (BL 2) added for those with cerebrovascular disease, and Zhigou (TE 6) added for those with digestive disease and constipation, and Hegu (LI 4) added for those with respiratory disease. Ear-point embedding method on Diaphragm(MA), Stomach(MA), Ershenmen(MA-TF 1), Subcortes(MA-AT1) and Kidney(MA) were conducted. Results showed cure in 45 cases, improvement in 10 cases and the effective rate was 100%.

ABSTRACT

Objective: To investigate the clinical efficacy of combined acupuncture and herbal medicine based on Prof. QIN Liang-fu's experience in treating chronic gastritis. Methods: Ninety chronic gastritis patients were randomly divided into acupuncture, herbal medicine and acupuncture plus herbal medicine groups. The clinical effects were evaluated after 6 months. Results: After 6months, the clinical effect was better in the acupuncture plus herbal medicine group than in the acupuncture group and in the herbal medicine group(P< 0.05). Conclusion:The curative effect on chronic gastritis was better in the acupuncture plus herbal medicine group than in the acupuncture group and in the herbal medicine group.
